OFA PROCEDURE

1) Do NOT feed your pet for 12 hours before your appointment- even though we do most animals without any form of sedation, occasionally it is required. Should the need arise, this will be discussed with you and an estimate will be provided.

2) Bring your registration papers (AKC/CKC/UKC)

3) OFA requires a permanent identification such as a tattoo or microchip- if your pet is not micro-chipped we can do that for you at your appointment, cost is $35.55.

4) In order for your pet to receive a permanent number from OFA it must be 2 years or older.

5) FEES:  This is for RADIOGRAPHS ONLY (this does NOT include OFA submission and mailing fees)
              Hips--$95
              Elbows--$85

              10% discount on three or more dogs in the same day

**Additional Fees**
OFA has submission fees for hips ($35), elbows ($35), hips and elbows together ($40)

We suggest that the owners mail the radiographs themselves and include the appropriate fees to OFA directly.

If you are requesting that we submit the radiographs for you must pay the OFA submission fee as well as a $15 mailing fee.
